- Release note titles are cut on a word boundary with no trailing
ellipsis or half-finished issue reference, and repeated issue
references are removed from index lines. 
- Commits without an issue
number are now added to the bottom of that day's existing note
instead of creating a separate unlinked page, so the index no longer
accumulates orphaned entries.
